За последние 10 лет отмечается увеличение количества тромбозов у госпитализированных детей на 130 %. Асимптоматический тромбоз глубоких вен (аТГВ) является случайной находкой, а данные о его частоте встречаемости, характеристиках и осложнениях зачастую представлены совместно с симптоматическими тромбозами, поэтому проблема бессимптомных тромбозов остается недостаточно изученной.</p><p>Цель исследования – изучить характеристики пациентов с асимптоматическим тромбозом, исходы тромбозов, а также частоту, степень тяжести и факторы риска посттромботического синдрома (ПТС) у таких пациентов. Оценить влияние терапии на реканализацию и развитие ПТС после аТГВ у детей.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было проведено моноцентровое ретроспективное исследование, в котором приняли участие 48 пациентов с аТГВ верхней или нижней конечности в возрасте от рождения до 17 лет (на момент развития тромбоза). Все пациенты наблюдались амбулаторно или получали стационарное лечение в НМИЦ ДГОИ им. Дмитрия Рогачева за период с 2013 по 2023 г. Пациенты были разделены на 2 группы: дети, которые не получали антикоагулянтную терапию (группа 1), и пациенты, которым данная терапия проводилась (группа 2). Далее у них были проанализированы частота развития, степень тяжести и факторы риска ПТС. Оценка ПТС проводилась в ходе плановых визитов больных к гематологу в НМИЦ ДГОИ им. Дмитрия Рогачева или с помощью интервью по телефону. Для диагностики ПТС использовались русифицированные варианты опросников Manco– Jonson Instrument (MJI), Modified Villalta Scale (MVS), the Clinical Assessment of Post-Thrombotic Syndrome (CAPTSure). Наличие статистически значимой разницы между двумя группами проверялось при помощи анализа таблиц сопряженности критерием хи-квадрат или, если в таблице был один из показателей меньше 5, точным тестом Фишера.</p></sec><sec><title>Результаты</title><p>Результаты. Большинство тромбозов (96 %) локализовались в системе верхней полой вены, 27 % тромбозов в исследовании – окклюзионные. Между группами пациентов, которые не получали терапию и которые ее получали, не отмечалось статистически значимых различий по возрасту, полу, наличию реканализации (полной или частичной) в исходе тромбоза, а также по частоте ПТС. Общая частота ПТС составила 46 % по шкале MVS/MJS и 29 % – по шкале CAPTSure. У большинства пациентов с ПТС отмечалось легкое течение, самым частым симптомом было наличие коллатералей. У 1 больной из группы 2 был среднетяжелый ПТС по шкале CAPTSure.</p></sec><sec><title>Заключение</title><p>Заключение. Общая частота ПТС у детей после аТГВ составила 46 % при оценке хотя бы по одной из шкал. Большинство случаев представлены легкой формой и имеют минимальные клинические симптомы. Не было статистически достоверных различий по реканализации тромбоза спустя 3 мес между группами пациентов, которые получали антикоагулянтную терапию и без нее. Также не отмечено значимых различий по частоте и тяжести ПТС между группами.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Over the past 10 years, there has been a 130 % increase in the amount of thrombosis in hospitalized children. The data about frequency, outcomes and long-term complications of incidental thrombosis is usually presented together with symptomatic thrombosis, and the problem of asymptomatic thrombosis seems to be underestimated.</p><p>Aim to the study – to assess the patient-related characteristics, outcomes of incidental thrombosis in children. To estimate the frequency, severity and risk factors of post-thrombotic syndrome (PTS) after incidental thrombosis. To evaluate the effect of therapy on thrombus resolution and frequency of PTS following asymptomatic deep vein thrombosis (DVT) in children.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We have retrospectively analyzed the medical records of 48 patients from 0 to 17 years (at the time of thrombosis) from 2013 to 2023 of our tertiary care Centre with objectively confirmed asymptomatic DVT of upper or lower extremity. All patients were divided into 2 groups: patients, who did not receive antithrombotic treatment (G1) and who assessed therapy (G2). Then we analyzed the data of PTS assessment in these patients. PTS was measured using Manco–Jonson Instrument (MJI), Modified Villalta Scale (MVS), the Clinical Assessment of Post-Thrombotic Syndrome (CAPTSure). Chi-square test was used to assess the statistical significance of differences between groups.</p></sec><sec><title>Results</title><p>Results. 90 % of patients were cancer patients, most of DVTs (96 %) were in the veins of upper extremity, 27 % of thrombosis were occlusive. There were no statistically significant differences between G1 and G2 in thrombus resolution, severity and frequency of PTS. The overall frequency of PTS was 46 % using MVS/MJI and 29 % using CAPTSure. Majority of children had mild PTS, the most common symptom was the presence of collaterals. There was 1 patient with moderate PTS in G2.</p></sec><sec><title>Conclusion</title><p>Conclusion. The overall frequency of PTS in patients with asymptomatic thrombosis was 46 %. However, most patients had mild PTS and slight clinical sings. There were no statistically significant difference in the rate of recanalization at 3 months after thrombosis between patients who received anticoagulant therapy and those who did not.
