Поздняя витамин К-зависимая коагулопатия считается успешно профилактируемым состоянием, однако современные данные о заболеваемости в группе детей, получивших профилактическое введение менадиона натрия бисульфита, отсутствуют, а эффективность самого препарата вызывает сомнения. При этом несвоевременная диагностика этого заболевания нередко приводит к инвалидизации и даже гибели пациентов.</p><p>Цель исследования – демонстрация высокой распространенности поздней витамин К-зависимой коагулопатии в группе здоровых доношенных детей, получивших профилактическое введение менадиона натрия бисульфита.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В ретроспективный анализ были включены все дети с установленным диагнозом поздняя витамин К-зависимая коагулопатия, проходившие лечение в ГБУ РО ОДКБ. У всех пациентов проводилась оценка данных анамнеза, необходимое лабораторное обследование. Диагноз поздняя витамин К-зависимая коагулопатия устанавливался на основании следующих критериев: появление спонтанных кровотечений любой локализации с 8-го дня по 6-й месяц жизни, наличие характерных изменений в коагулограмме (удлинение протромбинового времени, активированного частичного тромбопластинового времени, снижение протромбинового индекса) при отсутствии данных за наличие наследственной коагулопатии.</p></sec><sec><title>Результаты</title><p>Результаты. Диагноз поздняя витамин К-зависимая коагулопатия был установлен 13 детям, 11 из которых получили профилактическое введение менадиона натрия бисульфита в роддоме. У 4 пациентов имелись сопутствующие заболевания. Из 8 здоровых доношенных новорожденных, получивших профилактическое введение препарата, у 3 развились спонтанные внутричерепные кровоизлияния, 1 из пациентов погиб. Во всех случаях после введения менадиона натрия бисульфита показатели коагулограммы нормализовались.</p></sec><sec><title>Заключение</title><p>Заключение. Профилактическое введение менадиона натрия бисульфита, вероятно, не имеет достаточной эффективности в профилактике поздней витамин К-зависимой коагулопатии. Дальнейшее исследование эффективности этого препарата позволит модифицировать существующие в нашей стране подходы к профилактике и лечению этого заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Late-onset vitamin K deficiency bleeding is considered a successfully preventable condition, however, current data on morbidity in the group of children who received prophylactic administration of menadion are missing, and the effectiveness of the drug is questionable. At the same time, late diagnosis of this disease often leads to disability and death of patients.</p><p>The purpose of the study – to demonstrate the high prevalence of late vitamin K-dependent coagulopathy in a group of healthy full-term infants who received prophylactic administration of menadione sodium bisulfite.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The retrospective analysis included all children who had an established diagnosis of late-onset vitamin K deficiency bleeding and were treated at the Rostov Regional Children’s Clinical Hospital. All patients underwent an assessment of their medical history and necessary laboratory examinations. The diagnosis was made on the basis of the following criteria: spontaneous bleeding occurring between day 8 and month 6 of life; and characteristic changes in the coagulation parameters, with no evidence of hereditary coagulopathies.</p></sec><sec><title>Results</title><p>Results. The diagnosis of late-onset vitamin K deficiency bleeding was established in 13 children. 11 of them received prophylactic menadione administration in the hospital. Out of the 8 full-term healthy children who received the prophylaxis, 3 developed spontaneous intracranial hemorrhages and one patient died. In all cases, after the administration of menadione, coagulation parameters returned to normal.</p></sec><sec><title>Conclusion</title><p>Conclusion. Prophylactic administration of menadione is probably not effective enough in preventing late-onset Vitamin K deficiency bleeding.
