Применение органосохраняющих методик стало прорывом в лечении ретинобластомы (РБ). Cуперселективная интраартериальная химиотерапия (СИАХТ) относится к методам локальной химиотерапии. В настоящее время может применяться в 1-й линии терапии, особенно при распространенных односторонних опухолях, и несет меньший риск для системной токсичности. СИАХТ была признана безопасной процедурой с небольшим количеством осложнений. Однако впоследствии стали появляться сообщения о неблагоприятных кардиореспираторных нарушениях (КРН), таких как снижение комплайнса легких (податливости), брадикардия и последующая гемодинамическая нестабильность. Данное жизнеугрожающее состояние представляется как вегетативный кардиореспираторный рефлекс или тригеминокардиальный рефлекс. До сих пор не найден безопасный и эффективный метод их полного устранения. Его поиск является важным предметом исследований.</p><p>Цель исследования – обобщить текущие литературные данные по этиологии, патогенезу, диагностике, лечению и профилактике КРН, возникающих при проведении СИАХТ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнены поиск и анализ литературных данных в медицинских информационных системах PubMed, Сochranelibrarу, Cyberleninka, Google Scholar за последние 15 лет с использованием следующих ключевых слов: РБ, СИАХТ, КРН, кардиореспираторный рефлекс, анестезия при селективной внутриартериальной химиотерапии. Критериям поиска удовлетворяли 25 публикаций. В дополнение к этому ссылки на выбранные статьи были вручную проверены на наличие применимых статей, включающих последние сообщения о развитии КРН при проведении СИАХТ, помимо работ, имеющих историческое значение.</p><p>Материал предназначен для широкой аудитории врачей-анестезиологов-реаниматологов, онкологов, офтальмологов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The use of organ-preserving techniques has become a breakthrough in the treatment of retinoblastoma. Superselective intra-arterial chemotherapy (SIAC) belongs to the methods of local chemotherapy. It can currently be used in first-line therapy, especially for advanced unilateral tumors, and carries a lower risk for systemic toxicity. SIAC was recognized as a safe procedure with few complications. However, adverse cardiorespiratory dysfunction (CRD) such as decreased lung compliance, bradycardia and subsequent hemodynamic instability have subsequently been reported. This life-threatening condition presents as autonomic cardiorespiratory reflex or trigeminocardiac reflex. So far, no safe and effective method for their complete elimination has been found. Finding such a method is an important subject of research.</p><p>Purpose of the study – to summarize current literature data on etiology, pathogenesis, diagnosis, treatment and prevention of CRD arising during SIAC. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. We searched and analyzed the literature data in the medical information systems PubMed, Chranelibrary, Cyberleninka, Google Scholar for the last 15 years using the following key words: retinoblastoma, SIAC, CRD, cardiorespiratory reflex, anesthesia during SIAC. 25 publications met the search criteria.
