Современные успехи риск-адаптированной химиотерапии позволяют достигнуть многолетней общей выживаемости у 97,7 % детей с классической лимфомой Ходжкина (кЛХ) даже при распространенных стадиях заболевания. Тем не менее рецидивы и рефрактерное течение кЛХ отмечены в 10 % случаев. В связи с этим продолжается поиск дополнительных факторов прогноза течения ЛХ.</p><p>Цель исследования – оценить значимость применения индекса лейкоцитарного соотношения для оценки эффективности противоопухолевого лечения у детей пубертатного возраста, больных кЛХ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены дети обоих полов (22 мальчика и 18 девочек) с морфоиммуногистохимически подтвержденным диагнозом кЛХ. До лечения и после каждого курса химиотерапии определяли показатели общего анализа крови с последующим подсчетом лейкоцитарного соотношения по формуле: абсолютное количество нейтрофилов, деленное на абсолютное количество лимфоцитов. В качестве показателей нормы использовали результаты исследования значения лейкоцитарного соотношения 40 условно здоровых детей пубертатного возраста в равном половом числе. Статистическую обработку результатов проводили с использованием программы STATISTICA 10.0.</p></sec><sec><title>Результаты</title><p>Результаты. У всех больных детей значения лейкоцитарного соотношения превышали показатели условно здоровых детей от 2,4 до 4,4 раза, при этом наблюдалось возрастание значения лейкоцитарного соотношения в 1,7 раза у пациентов с распространенными (III, IV) стадиями опухолевого процесса в сравнении со II стадией. Ретроспективный анализ значений лейкоцитарного соотношения у детей обоих полов с ЛХ с достигнутой ремиссией за счет 1-й линии терапии до лечения продемонстрировал повышение значения лейкоцитарного соотношения в 3,5 раза у мальчиков и в 4,1 раза у девочек. На всех этапах лечения с 1-го по 6-й курсы выявлено статистически значимое снижение изученного показателя по сравнению со значением показателя до лечения у мальчиков в среднем в 3,6 раза, а у девочек в среднем в 4,3 раза. Показатель был в пределах референсных значений нормы. Ретроспективный анализ значений лейкоцитарного соотношения у детей пубертатного возраста с рефрактерным и прогрессирующим течением кЛХ (4 мальчика и 3 девочки) выявил, что до лечения в сравнении с нормой среднее значение было выше в 2,6 раза у мальчиков, в 4,2 раза у девочек. В сравнении с инициальными показателями значений лейкоцитарного соотношения у детей пубертатного возраста с кЛХ и достигнутой ремиссией за счет 1-й линии терапии полученные данные статистически незначимы. Проанализировав значения лейкоцитарного соотношения после каждого курса полихимиотерапевтического лечения, мы выяснили, что показатель статистически значимо повышался после тех курсов, на которых клинически и диагностически (ультразвуковое исследование, компьютерная томография (КТ), позитронно-эмиссионная томография (ПЭТ)/КТ) выявлялись рефрактерное течение или прогрессия заболевания. Показатель лейкоцитарного соотношения реагировал изменением значения в зависимости от эффекта последующих линий терапии.</p></sec><sec><title>Заключение</title><p>Заключение. Помимо объективных инструментально-лабораторных методов оценки эффективности специализированной терапии (ПЭТ/КТ с 18F-фтордезоксиглюкозой, биопсия остаточного новообразования) в качестве дополнительного способа у детей пубертатного возраста с кЛХ возможно также использование индекса лейкоцитарного соотношения по результатам общего анализа крови, что позволяет косвенно судить об эффективности проводимой терапии и своевременно выявлять рефрактерное течение или прогрессирование кЛХ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Modern advances in risk-adapted chemotherapy allow achieving long-term overall survival in 97.7 % of children with classical Hodgkin lymphoma (cHL), even at widespread stages of the disease. However, relapses and refractory course of cHL are noted in 10 % of cases. In this regard, the search for additional prognostic factors for the course of HL continues.</p><p>The aim of the study – to evaluate the significance of using the leukocyte ratio index to assess the effectiveness of antitumor treatment in pubertal children with cHL.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included children of both genders (22 boys and 18 girls) with a morphoimmunohistochemically confirmed diagnosis of cHL. Before treatment and after each course of chemotherapy, the general blood test parameters were determined with subsequent calculation of the leukocyte ratio using the formula: the absolute number of neutrophils was divided by the absolute number of lymphocytes. The results of the study of the leukocyte ratio of 40 conditionally healthy pubertal children in equal sex numbers were used as normal indicators. Statistical processing of the results was performed using the STATISTICA 10.0 program.</p></sec><sec><title>Results</title><p>Results. In all children with cHL, the leukocyte ratio values exceeded the values of conditionally healthy children from 2.4 to 4.4 times, while there was an increase in the leukocyte ratio by 1.7 times in children with advanced (III, IV) stages of the tumor process compared to stage II. Retrospective analysis of the leukocyte ratio values in children of both genders with HL with remission achieved due to the first line of therapy before treatment demonstrated an increase in the leukocyte ratio by 3.5 times in boys and 4.1 times in girls. At all stages of treatment from the 1st to the 6th course, a statistically significant decrease in the studied indicator was revealed compared to the value of the indicator before treatment in boys by an average of 3.6 times, and in girls by an average of 4.3 times. The indicator was within the reference values of the norm. Retrospective analysis of the leukocyte ratio values in adolescent children with refractory and progressive cHL (4 boys and 3 girls) revealed that before treatment, compared to the norm, the average value was 2.6 times higher in boys and 4.2 times higher in girls. Compared with the initial values of the leukocyte ratio in adolescent children with cHL and remission achieved due to the first line of therapy, the data obtained are statistically insignificant. Having analyzed the leukocyte ratio values after each course of polychemotherapy for each patient in particular, it turned out that the indicator statistically significantly increased after those courses in which refractory course or progression of the disease was detected clinically and diagnostically (ultrasound examination, CT, PET/CT). The leukocyte ratio indicator responded by changing the value depending on the effect of subsequent lines of therapy.</p></sec><sec><title>Conclusions</title><p>Conclusions.
