Опухолевый тромбоз магистральных сосудов является редким осложнением при злокачественных новообразованиях у детей.

В мировой литературе отмечены лишь единичные наблюдения тромбоза магистральных сосудов при гепатобластоме. Это ставит перед хирургом задачи, решение которых не имеет единого и стандартизированного подхода, а также отсутствует достаточный клинический опыт в педиатрической практике. Лечение пациентов с подобными осложнениями требует тщательной подготовки, планирования и слаженной работы междисциплинарной группы специалистов. В данной статье представлено клиническое наблюдение ребенка с гепатобластомой правой доли печени с опухоль-ассоциированным тромбозом правой печеночной и нижней полой вен, распространившимся до уровня правого предсердия. После неоадъювантной полихимиотерапии пациенту выполнена правосторонняя гемигепатэктомия с резекцией нижней полой вены на уровне гепатикокавального конфлюенса и последующей реконструкцией с использованием аутовставки.

Tumor thrombosis of the main vessels is a rare complication of malignant neoplasms in children. In the world literature, a few clinical cases of thrombosis of the main vessels in hepatoblastoma are noted. This poses questions to the surgeon, the solution of which does not have a single and standardized approach, as well as sufficient clinical experience in pediatric practice. Treatment of patients with such complications requires careful preparation, planning and coordinated work of an interdisciplinary team of specialists. This article presents a clinical case of a child with right liver lobe hepatoblastoma with tumor-associated thrombosis of the right hepatic vein and inferior vena cava, extending to the right atrium. After neoadjuvant chemotherapy, the patient underwent right extended hepatectomy with resection of the inferior vena cava at the level of hepaticocaval confluence and subsequent reconstruction using an autogenic patch.
