В связи с отсутствием у большинства российских пациентов совместимого по HLA-генам родственного и неродственного донора в Международном регистре доноров костного мозга актуален поиск альтернативных источников для проведения алло-ТГСК. Трансплантация от частично совместимого по HLA-генам родственного донора (гаплоидентичного) (гапло-ТГСК) – быстрый и универсальный метод лечения. В исследование включено 56 детей и подростков в возрасте от 1 до 21 года (медиана возраста – 9 лет) с резистентными формами острого лимфобластного лейкоза (ОЛЛ) (n = 32) и острого миелобластного лейкоза (ОМЛ) (n = 24), получивших гапло-ТГСК в период с декабря 2007 по июнь 2013 г. В режиме предтрансплантационной подготовки и профилактики острой реакции «трансплантат против хоязина» (РТПХ) использовали  иелоаблативный режим кондиционирования (МАК) + антитимоцитарный глобулин (АТГАМ) (n = 20), МАК + циклофосфан (ЦФ) (n = 5), режим со сниженной интенсивностью доз (РИК) + АТГАМ (n = 14), РИК + Кэмпас (n = 4), РИК + ЦФ (n = 13). Все пациенты получали базовую иммуносупрессивную терапию такролимусомили циклоспорином А. В качестве гапло-трансплантата применяли: комбинацию стимулированных гранулоцитарным колониестимулирующим фактором (Г-КСФ) неманипулированного костного мозга (КМ) и периферических стволовых клеток крови (ПСКК) с позитивной селекцией CD34+-клеток (n = 23) или стимулированный Г-КСФ неманипулированный КМ (n = 33). Общая выживаемость (ОВ) в группе больных составила 33,3 %. Статистически достоверно лучшая ОВ (р = 0,03) отмечалась у пациентов, трансплантированных стимулированным Г-КСФ неманипулированным КМ – 45,5 % против 13 % при использовании комбинированного трансплантата. Также в статье описаны другие факторы, оказавшие влияние на результаты гапло-ТГСК (возраст, статус на момент гапло-ТГСК, используемый режим кондиционирования, применение посттрансплантационной терапии, восстановление кроветворения, клеточность трансплантата).</p></abstract><trans-abstract xml:lang="en"><p>Allogenic hematopoietic stem cell transplantation (allo-HSCT) is one of the effective methods of treatment of patients with malignant disorders of hematopoietic system. Because of absence of related and unrelated donors at the majority of Russian patients, search for alternative grafts for allo-HSCT is actual now. HSCT from partially HLA-matched related (haploidentical, haplo-HSCT) is the fast and universal method of treatment. Fifty six children and young adults 1–21 years old (age median – 9 years old) with resistant forms of acute lymphoblastic leukemia (ALL) (n = 32) and acute myeloid leukemia (AML) (n = 24) were transplanted with haploidentical donor from December 2007 to June 2013. In preparative therapy and graft-versus-host disease (GvHD) prevention the following regimens were used: myeloablative conditioning regimen (MAC) + Antithymocyte Immunoglobulin (ATGAM) (n = 20), MAC + Cyclophosphamide (CpH) (n = 5), reduce-intensity regimen (RIC) +ATGAM (n = 14), RIC + Alemtuzumab (n = 4), RIC + CpH (n = 13). All patients received base immunosuppressive therapy with Tacrolimus and Cyclosporin A. The following types of haplo-transplants were used: combination of stimulated with G-CSG unmanipulated bone marrow (BM) and peripheral blood stem cells (PBSC) with positive CD34+ selection (n = 23) or stimulated with G-GSF unmanipulated BM (n = 33). Overall survival (OS) in this group of patients was 33.3 %. Statistically better OS (р = 0.03) was revealed in a group of patients transplanted with G-CSF stimulated unmanipulated BM – 45.5 % versus 13 % in case of combined transplant.
