Особенную категорию представляют пациенты, которым необходима расширенная резекция, грозящая развитием пострезекционной печеночной недостаточности. При недостаточном объеме будущего остатка печени методом выбора является двухэтапная резекция или трансплантация печени. Несмотря на широкое распространение лапароскопической хирургии печени у взрослых, в педиатрической практике минимально инвазивный доступ не изучен, а литературные данные, посвященные этому вопросу, скудны. В настоящей публикации представлен успешный опыт ALPPS у ребенка, которому 1-й этап операции впервые выполнен полностью лапароско-пически. Описанное наблюдение подтверждает возможность проведения лапароскопических резекций печени у детей и эффективность частичного разделения паренхимы печени для достижения адекватного прироста объема ее будущего остатка в целях избежания трансплантации и ее осложнений.</p></abstract><trans-abstract xml:lang="en"><p>Radical tumor removal remains the key task in hepatoblastoma surgery. Patients who need extended resection and have a risk of posthepatectomy liver failure are particularly difficult. In cases with insufficient future liver volume, the method of choice is a two-staged hepatectomy or liver transplantation. Despite the wide spread of laparoscopic liver surgery in adults, the minimally invasive approach has not been studied in pediatrics, and the literature on this issue is scarce. In current publication a case of successful partial associating liver partition with portal vein ligation for staged hepatectomy (ALPPS) in a child, firstly accomplished pure laparoscopically.
