Лимфома Ходжкина (ЛХ) является редким клональным злокачественным заболеванием лимфатической системы. На долю ЛХ в структуре онкологической заболеваемости у детей приходится 4–6 %, у подростков – 16 %. В настоящее время пациенты детского и подросткового возраста с морфологически верифицированной ЛХ в условиях отделения детской онкологии и гематологии ГБУЗ «Морозовская ДГКБ ДЗМ» получают программную терапию в рамках протокола GPOH-HD-2003. Авторами статьи представлен опыт лечения детей и подростков с ЛХ согласно протоколу GPOH-HD-2003 в условиях отделения онкологии и гематологии ГБУЗ «Морозовская ДГКБ ДЗМ».</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. С 1 января 2003 г. по 31 декабря 2018 г. 62 пациента (35 мальчиков, 27 девочек) с ЛХ получали программную терапию в рамках протокола GPOH-HD-2003. Возраст пациентов до начала специфической терапии варьировал от 4,7 до 17,6 года, медиана возраста составила 13,3 года. Все пациенты были стратифицированы на терапевтические группы (ТГ) в зависимости от стадии заболевания, в ТГ-1 входили 2 ребенка, в ТГ-2 – 30, в ТГ-3 – 30 детей. Все пациенты ТГ-1 получали 2 индукционных цикла: ОРРА для девочек, ОЕРА для мальчиков. Пациенты ТГ-2 и ТГ-3 – 2 или 4 курса соответственно: СОРР (девочки), COPDAC (мальчики). После окончания полихимиотерапии все пациенты проходили лучевую терапию в дозе 19,8 Гр на инициально пораженные области.</p></sec><sec><title>Результаты</title><p>Результаты. Наиболее частым осложнением после курсов ОЕРА у мальчиков была нейтропения I–II степени, после ОРРА у девочек – нейтропения III–IV степени. При анализе миелотоксичности курсов COPDAC и СОРР выяснилось, что курсы СОРР более токсичны по сравнению с COPDAC. Показатели общей (ОВ), бессобытийной (БСВ) и безрецидивной (БРВ) выживаемости за все время наблюдения составили 100 % (медиана наблюдения – 48,12 мес). В указанный период не было зафиксировано ни одного случая летальности, обусловленного проведением терапии, рецидива ЛХ или вторничных опухолей.</p></sec><sec><title>Заключение</title><p>Заключение. Показатели 5-летних ОВ, БРВ, БСВ свидетельствуют о высокой эффективности и низкой токсичности программной терапии ЛХ по протоколу GPOH-HD-2003 вне зависимости от стадии заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Hodgkin’s lymphoma (HL) is a rare clonal neoplasm of lymphatic tissue. In the structure of malignant neoplasms in children HL accounts for 4–6 % and for 16 % in adolescents. Now children and adolescents with confirmed histology of HL in the Department of Oncology and Hematology, Morozovskaya Children’s Clinical Hospital, treated with the GPOH-HD-2003 protocol. The authors of the article presented the experience of treatment children and adolescents with HL according to recommendations of GPOH-HD-2003 in the Department of Oncology and Hematology, Morozovskaya Children’s Clinical Hospital.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. From January 1, 2003 until December 31, 2018, 62 patients (35 boys, 27 girls) received therapy according to recommendations of GPOH-HD-2003 study. Age of patients ranges from 4.7 to 17.6 years, mean age was 13.3 years. All patients are allocated to three treatment groups (TGs) based on staging, in TG-1 were 2 patients, in TG-2 – 30 patients, in TG-3 – 30 patients. TG-1 patients received two courses of for induction: OEPA for boys and OPPA for girls. TG-2 and TG-3 patients received further two or four cycles COPP (girls) or COPDAC (boys), respectively. After chemotherapy all patients received initially involved-field irradiation with 19.8 Gy.</p></sec><sec><title>Results</title><p>Results. CTCEA grade 1 or 2 hematotoxicity was the most common recorded adverse reaction in the OEPA regimen for boys, grade 3 or 4 hematotoxicity – in the OPPA regimen for girls. Also hematotoxicity was more pronounced with COPP than COPDAC. Overall (OS), event-free (EFS) and relapse-free (RFS) survival rates at 5 years were 100 % for the period of the observation time (the median observation time was 48.12 months). No treatment-related deaths, relapses or second malignancies were recorded during this period.</p></sec><sec><title>Conclusion</title><p>Conclusion.
