Введение. Ретинобластома (РБ) – это злокачественное новообразование, которое является наиболее распространенной внутриглазной опухолью детского возраста во всем мире. С появлением внутриартериальной химиотерапии интервенционные хирурги взяли на себя центральную роль в лечении этого детского заболевания. Внутриартериальная химиотерапия – это новый метод лечения РБ, при котором химиотерапевтические агенты точно доставляются в глазную артерию, сводя к минимуму системную токсичность. Эта процедура показала впечатляющие результаты и позволила значительно снизить частоту энуклеации при распространенной и рефрактерной РБ. Однако процедура влечет за собой потенциально серьезные острые респираторные и гемодинамические нарушения.

Цель исследования – представить опыт проведения и особенности анестезиологического обеспечения при развитии жизнеугрожающих состояний при суперселективной интраартериальной химиотерапии (СИАХТ) у 2 пациентов с РБ.

Материалы и методы

Материалы и методы. Представлены клинические случаи 2 пациентов в возрасте 2 лет с РБ, которые получили 3 курса СИАХТ по поводу заболевания с развитием тяжелой тригемино-кардиальной реакции.

Заключение

Заключение. СИАХТ является одним из новых перспективных методов лечения РБ. Выраженные кардиореспираторные осложнения часто наблюдаются во время проведения общей анестезии при повторных сеансах СИАХТ и могут быть потенциально опасными для жизни. Предположительно, данные осложнения представляют собой вегетативный рефлекторный ответ на катетеризацию глазной артерии. Поэтому все пациенты с РБ, которым планируется проведение СИАХТ, должны быть включены в группу повышенного риска. Время возникновения тригемино-кардиального рефлекса предсказуемо, носит временный характер, но анестезиолог должен быть готов к купированию развившегося осложнения с помощью препаратов для экстренной помощи (адреналин).

Introduction

Introduction. Retinoblastoma (RB), a malignant neoplasm, is the most common pediatric intraocular tumor worldwide. With the advent of intra-arterial chemotherapy, interventional surgeons have assumed a central role in the treatment of this pediatric disease. Intra-arterial chemotherapy is a new treatment modality for RB in which chemotherapeutic agents are precisely delivered into the ocular artery, minimizing systemic toxicity. This procedure has shown impressive results and has significantly reduced the rate of enucleation in advanced and refractory RB. However, the procedure entails potentially serious acute respiratory and hemodynamic disturbances.

Purpose of the study – present our experience with and features of anesthesia management in the development of life-threatening conditions during superselective intraarterial chemotherapy (SIAC) in two patients with RB.

Materials and methods

Materials and methods. We present clinical cases in 2 2-year-old patients with RB who received three courses of SIAC for RB with development of severe trigeminocardial reaction.

Conclusion

Conclusion. SIAC is one of the new promising treatments for RB. Prominent cardiorespiratory complications are frequently observed during general anesthesia for repeated sessions of SIAC and can be potentially life-threatening. Presumably, these complications represent an autonomic reflex response to ocular catheterization. Therefore, all patients with RB who are scheduled for SIAC should be included in the high-risk group. The timing of occurrence of the trigeminocardiac reflex is predictable and temporary, but the anesthesiologist must be prepared to treat the developed complication with the help of emergency drugs (adrenaline).
