Оценка качества жизни пациентов, перенесших противоопухолевую терапию злокачественных новообразований центральной нервной системы в детском возрасте

Аннотация
Актуальность. Согласно данным международной статистики, опухоли центральной нервной системы занимают лидирующие позиции в структуре детской онкологической заболеваемости. Применение мультимодальной стратегии противоопухолевой терапии на сегодняшний день позволяет достичь высокого уровня долгосрочной выживаемости, но зачастую с тяжелыми вариантами отдаленной токсичности. Изучение вопросов состояния здоровья излеченных пациентов и оценка их качества жизни (КЖ) в аспектах поиска управляемых факторов снижения токсичности лечения является актуальной проблемой детской онкологии и педиатрии.
Материалы и методы. В исследование включены 76 пациентов с подтвержденными диагнозами медуллобластомы (93,4 %) или атипичной тератоидно-рабдоидной опухоли (6,6 %), установленными в возрасте до 18 лет, по поводу которых проводилась противоопухолевая терапия. Для оценки КЖ использовался опросник Health Utilities Index версий Mark 2 и 3 (HUI2 и HUI3), состоящий из 15 вопросов. Было проанализировано влияние бремени болезни (гистологический тип опухоли, стадия и тип (первичная опухоль/рецидив) заболевания, включение высокодозной химиотерапии, объем лучевой терапии и операции) на показатели здоровья.
Результаты. Медиана возраста пациентов на момент анкетирования составила 11,7 года с медианой интервала после завершения специфического лечения 6,4 года. Респондентами в анкетировании являлись родители пациентов (n = 37; 48,7 %) и/или лечащий врач, осуществляющий медицинское наблюдение за ребенком (n = 39; 51,3 %). Среднее время, потраченное на заполнение опросника, составило 8,0 ± 3,0 мин. Частота снижения уровней составляющих здоровья по HUI2 и HUI3 достигла 47 % и 53 %, соответствуя легкой (66 % и 62 %), умеренной (27 % и 26 %) и тяжелой (7 % и 12 %) степеням нарушений. Функциональные нарушения здоровья в наибольшей степени касались таких составляющих КЖ, как когнитивные способности, эмоции, ощущение, подвижность, варьируя от умеренных до тяжелых. Статистически значимое негативное влияние на двигательные способности (p = 0,02), самообслуживание (p = 0,05) и мелкую моторику (p = 0,01) имела метастатическая стадия опухоли, у пациентов с рецидивами заболевания выявлено снижение когнитивного (p = 0,03) и эмоционального (p = 0,002) статусов, применение краниоспинального облучения было также ассоциировано со снижением когнитивных способностей (p = 0,04).
Выводы. Оценка КЖ пациентов с использованием HUI2 и HUI3 является простым и доступным методом для рутинного применения в клинической практике. У детей, перенесших противоопухолевую терапию по поводу медуллобластомы или атипичной тератоидно-рабдоидной опухоли, зарегистрированы статистически значимые отклонения составляющих здоровья, при этом неблагоприятными факторами риска являлись рецидив, метастатическая стадия заболевания и лучевая терапия в объеме краниоспинального облучения. Важно отметить, что, несмотря на высокий процент зарегистрированных отклонений в оцениваемых составляющих здоровья, более половины из них соответствовали нарушениям легкой и умеренной степени. Наиболее уязвимыми являлись двигательная сфера и когнитивные способности. Учитывая малочисленность выборки, нельзя говорить о высокой степени значимости полученных результатов, что требует продолжения исследований в указанной когорте больных, тем не менее таковые совпадают с международными литературными данными. Следует также отметить, что анкетирование дает оценку состояния здоровья пациентов в конкретный период, в то время как оно может значительно меняться с течением времени и на фоне многочисленных факторов, тем самым определяя необходимость динамического мониторинга.
