Продолжение адъювантной химиотерапии при остеогенной саркоме, резистентной к неоадъювантному лечению. Осмысленные действия или привычка?

Аннотация
Химиотерапия (ХТ) значимо увеличила шанс на излечение больных с резектабельной остеогенной саркомой и сейчас является неотъемлемой частью их лечения. При этом стандартный подход, предусматривающий введение всем пациентам одних и тех же цитостатиков (доксорубицин, цисплатин, метотрексат) и перед, и после операции, остается неизменным на протяжении последних десятилетий. К сожалению, результаты его использования у больных с плохим ответом (патоморфозом) опухоли на предоперационную ХТ малоудовлетворительны, а недавно завершившиеся исследования не показали их улучшения за счет интенсификации послеоперационной ХТ. В результате стандартом лечения этих больных остается продолжение после операции ХТ, оказавшейся неэффективной на дооперационном этапе. Статья посвящена критическому анализу целесообразности подобной практики и оценке возможных позитивных и негативных последствий отказа от послеоперационной ХТ у больных с плохим патоморфозом опухоли.
