Abstract

Blinatumomab, a bispecific antibody construct targeting CD19, is the most advanced member of bis-pecific T-cell engager (BiTE®) molecules. The clinical development program includes B-precursor acute lymphoblastic leukemia (ALL) and B-cell non-Hodgkin lymphoma (NHL). Remissions induced in pediatric and adult patients with relapsed/refractory B-precursor ALL have allowed for successful allogeneic hematopoietic stem cell transplantation in this setting. Blinatumomab has also induced durable responses in low-grade B-cell NHL. Blinatumomab recently gained approval in the United States by the U.S. Food and Drug Administration for treatment of Philadelphia chromosome-negative B-precursor relapsed/refractory acute lymphoblastic leukemia. AMG 330 is an investigational anti-CD33 BiTE® antibody construct. Targeting CD33 ex vivo in primary samples from patients with acute myeloid leukemia (AML) has shown AMG 330-mediated T-cell expansion and T-cell cytotoxicity against AML cells.

Abstract

Palliative therapy for pediatric patients in Russian Federation is at stage of formation nowadays. Only in the April of previous year the Ministry of Health order on palliative care for children was issued. The separate existence of palliative care for children is indicated in this order. Oncological patients in terminal condition is about 20–30 % of all children require palliative care. But they always enter the palliative care program in severe situation after the treatment in oncological departments with a lot of medical problems. One of the main problems that determine quality of life of child after the decision on incurability is pain syndrome.

Abstract

About the 50 % of patients with neuroblastoma (NB) suffered from generalized form of disease at the moment of diagnosis establishing. So, the correct staging with the usage of methods of radiation diagnosis and nuclear medicine is very important in this setting. Meta-iodobenzyl guanidine (MIBG) is an analogue of noradrenalin so this is the ideal tumortropic drug for visualization of cells expresses carriers of noradrenalin. Main problems of method of MIBG scintigraphy are the limited spatial resolution, low sensitivity during the diagnosis of small-sized tumours, presence of NB without MIBG accumulation, duration of scanning and necessity of several scanning protocols. Positron-emission tomography with consequent radiopharmaceuticals can be good alternative way in comparison with routine scintigraphy with MIBG to help in case of doubtful and contradictory results.

Abstract

Estimation of glomelular filtration rate (GFR) has the special importance because it can provide the information on damage of glomerulus and can be the one of the major early criterions characterized the normal function of kidneys. Width-available methods of GFR estimation can not always give valid results. Methods with the usage of radiopharm drugs (RPD) allows to perform the estimation of GFR with the high accuracy that has the big clinical significance.

Aim of the work – to test the protocols of GFR estimation with the help of the clearance of single infused RPD 99mTc-DTPA and reveal the correlation between GFR estimated by the clearance of 99mTc-DTPA calculated with gamma spectrometric method (GS) and liquid scintillation spectrometry (LSS).

Twenty patients were enrolled. Blood samples collection was made after 1, 2 and 3 hours after intravenous infusion of 99mTc-DTPA. Estimation was made by the LSS method and with the help of GS. GFR was calculated as rate of infused dose D0 to surface under the curve of excretion S.

Our results suggest that the previous published data is correct. This data shows that for correct calculation of GFR is enough to do the estimation of drug clearance in blood samples received after 1, 2 and 3 hours after infusion of RPD.

Thus, the protocols of GFR estimation by the clearance of single infused RPD by the methods of GS and LSS were tested. GFR data calculated by 99mTc-DTPA clearance with the help of method LSS did not showed any differences in comparison with GS method.

Abstract

Background. Combination of total body irradiation (TBI) with chemotherapy is widely used technique for conditioning before hematopoietic stem cell transplantation for patient with hematological malignancies worldwide. Best results of patient’s survival were showed in comparison with conditioning regimen included only chemotherapy.

Aim – development of TBI technique at children with the help of TomoTherapy equipment and including it to the conditioning protocol before allogenic bone marrow transplantation.

Materials and methods. During the period between July 2014 and February 2016 radiation therapy was made for 38 patients. Treatment was performed daily, 2 times per day with single basic dose on extended target (PTV) 2 Gr to total focal dose 12 Gr. Taking into account potential sensitivity of healthy organs and based on literature data, the following risk organs were chosen: lungs, kidneys and anterior eye chamber.

Results. All patients successfully passed the planned program of treatment. Follow-up period was from 0 to 21 months (average – 7.2 months). No one of the patients did not showed any toxic episodes from the critical organs (lungs, kidneys, eye lens).

Conclusions. TBI on TomoTherapy equipment allows to provide maximal control on irradiation dose distribution in PTV with the decreasing of radiation for critical organs. This allows achieving the decreasing of possibility of serious complications of radiation therapy and high effectiveness of treatment.

Abstract

Allogeneic hematopoietic stem cell transplantation is an effective treatment for patients with hematological malignancies, inherited diseases and solid tumors. To perform the transplantation, you must have HLA-compatible bone marrow donor. Only 10–15 % of Russians can be found a related donor, which requires a search for alternative unrelated donor at the international registry. Search in the international registry is associated with significant financial costs and long term activation of the donor. Creation of a unified Russian registry of bone marrow donors and online data base – “Bone Marrow Donor Search” (www.bmds.info) will help to reduce the cost and increase the availability of allogeneic hematopoietic stem cell transplantation procedure for Russian citizens. First results of the donor bone marrow registry, based on Raisa Gorbacheva Memorial Research Institute of Children Oncology, Hematology and Transplantation has shown its effectiveness and relevance among transplant centers in Russian Federation and the Republic of Kazakhstan.

Abstract

This issue presents information on actual problem of estimation of nutrition status and providing of nutrition support for children with hematological and oncological diseases. High actuality of education of specialists and parents in this field is presented. Authors present the answers on questionnaires of centers of pediatric hematology and oncology about the effectiveness of round tables on nutrition support during the educational seminars of program “Far regions”. Answers of parents on questionnaire about the nutrition support are presented. These answers showed the significance of support and collaboration between parents and physicians for success.

Abstract

Multidisciplinary approach is widely used worldwide among the specialists of different specialties. Joint efforts of oncologist, hematologist, surgeon, radiologist, pathomorphologist, anesthesiologist, ICU-staff, recreation therapist and other specialists are aimed on treatment of patients with hematological and oncological diseases. This manuscript showed the view of physicians on place of surgery in this complex system. Progress of surgery with the help of endoscopic technique, intraoperational navigation, reconstructive and microscopic technique open the width range of possibilities in diagnosing and treatment of patients. Only harmonious work of all staff members from the moment of patient’s entering to clinic to the successful healing, work of clinical-scientific parties allows to reach good oncological and functional results, to prepare new methods and introduce it to the practical usage. This will allow the early social adaption of children and adolescents.
